When to seek urgent medical or dental care
Some problems go beyond a damaged brace or aligner. The American Association of Orthodontists advises people to seek urgent care for heavy or uncontrolled bleeding, trouble breathing or swallowing, a suspected broken jaw, major facial trauma, or a permanent tooth that has been knocked out or badly displaced.
Call 911 or go to an emergency department when an injury affects breathing, causes serious bleeding, or may involve the jaw or face. Contact a dentist promptly for an injured or knocked-out tooth. Your orthodontist should also know about the injury because braces, aligners, or other appliances may need to be checked.
Problems that usually start with a call to your orthodontist
Tell the office what you see and how you feel. A clear description and a photo, if the office requests one, may help the team decide what should happen next. Do not pull on a loose appliance or keep wearing something sharp, badly distorted, or painful unless your orthodontic team tells you to do so.

The AAO recommends contacting your orthodontist when a bracket is loose, a wire moves out of place, or discomfort is unusual. While you wait for instructions, orthodontic wax may cover a sharp area and reduce rubbing. Soft foods can also be easier to manage when the mouth feels tender.
